The genetic questionnaire must be completed by all obstetrical patients. The HIV screening, Triple Screen, and Cystic Fibrosis screen are informational sheets that must be read and signed by all obstetrical patients. The obstetrical reminders are helpful notes for each trimester of your pregnancy.
